Statement on attaching documents *

The entry is accompanied by scans of the horse's current registration documents. This entry is invalid without the contact details of the person entering the horse. The person responsible for the horse is its owner or leaseholder, but the person making the entry (presenter, other employees, e.g. stablemen or veterinarian) may be considered an additional responsible person if they are present at the show or make important decisions regarding the horse.

I hereby declare that I consent to the processing of my personal data specified in this form by the Polish Arabian Horse Breeders Association with its registered office in Janów Podlaski for the purposes of conducting the process of registering a horse for participation in the All-Polish Arabian Horse Championship show - organizing the show and creating a show catalog, as well as for the purposes of preparing accounting documents in connection with issuing VAT invoices. I also declare that I have been informed by the Polish Arabian Horse Breeders Association of the right to withdraw my consent to the processing of my personal data, and my withdrawal of such consent will not affect the lawfulness of the processing of my data, which the Polish Arabian Horse Breeders Association carried out before the withdrawal.
