Statement on attaching documents *

Scans of the horse's current registration documents are attached to the application. This entry is invalid without the contact details of the person registering the horse. The person responsible for the horse is its owner or lessee, but the person making the report (the presenter, other employees, e.g. stable staff or a veterinarian) may be considered an additional person responsible if he or she is present at the show or makes important decisions regarding the horse.

I hereby declare that I consent to the processing of my personal data indicated in this application form by the Polish Arabian Horse Breeders Association based in Janów Podlaski for the purposes of conducting the process of registering a horse to participate in the All-Polish Arabian Horse Championship (B-Nat) show - organization of the show and creating a show catalogue, and also for the purposes of preparing accounting documents in connection with issuing VAT invoices. I also declare that I have been informed by the Polish Arabian Horse Breeders Association about the right to withdraw consent to the processing of my personal data, and my withdrawal of such consent will not affect the lawfulness of the processing of my data, which was carried out by the Polish Arabian Horse Breeders Association before the withdrawal.
